Intro to Home Care Services

Our home is where our heart is. We live there, we sleep there we socialize there, we raise our children there and these days we almost do everything from there. It is our haven of comfort. However, there are times when our home can become very isolating and overwhelming when there is not enough support to help out when our health declines. In these circumstances it is important to have those who can help and support close at hand. For this reason, it is important to have the contacts of those who can help out should we need it (American Cancer Society). When these professionals can provide culturally sensitive care in the home in the daily life, life regains meaning (Sanerma et al., 2020).

Bloomhill has various services that can help out when you are feeling unwell. These include transport services, which can be used for transportation to and from treatment, or to and from your home and Bloomhill, cost of transport can be discussed with our transport team. These services can also help when there are specialist appointments in Brisbane. Bloomhill also has a number of Opshops that have various items that may be needed at home, and includes electric recliners, portable toilets and other small items. It you are struggling to find items please do reach out to the Bloomhill nurses as we may be able to loan items if available. Bloomhill also works in cooperation with other care organisations and can help with referrals.

When you feel overwhelmed, reach out, say something.  Bloomhill is here to help you throughout your journey in all stages. We have resources to help, and if not, we have the professional networks to support. According to Abusalem et al., (2012), care is usually provided by a multidisciplinary health service provider from which the individual is referred to home care services. Thus, the initial nursing needs assessment conducted at Bloomhill is essential to identify needs. This needs assessment is also essential for comprehensive planning, referral and needs implementation (Shahriari et al., 2024). The identification of these needs, the coordination of care, and the provision of these needs in the home-context is an important part of client-centred care as it enables self-determination, continuity of care and family-centeredness (Sanerma et al., 2020).
